Advanced Healthcare Directive (Living Will)

What is it?

A way to write down your healthcare decisions for the future so they must be followed. These documents tell your doctor and supporters what kind of medical treatment you want or do not want, in case you cannot tell them later. To complete a Healthcare Directive, you must understand the benefits, risks and possible healthcare choices and be able to make a decision and share your decision; lawyers call this capacity.

Some conservatorships cover healthcare decisions.

How do I learn more?

Tennessee has an Advance Healthcare Directive form. You do not need a lawyer to fill out an Advance Healthcare Directive.
